This online form must be completed by the parent or caregiver. If the personal credentials (valid driver’s license or social security card) cannot be provided, cookies must be paid for in advance. Due to council by January 2, 2025.
Product Program Manager Agreement (Digital | Print)
Volunteers who want or need access to eBudde must complete this online form, be a registered Girl Scout volunteer and have a current background check on file. Due to council by January 2, 2025.
Product Program Banking Authorization Form (Troop/SU ACH)
A signer of the troop account must complete this online form giving GSHG permission to credit or debit the troop account. Additional troop bank account documentation may be required if there has been any change in the leadership team or troop bank account since last year. Troop bank account information must be entered into eBudde.
Troop Debt Procedure
This digital form must be completed and signed by the person accepting the Troop Product Manager position as well as the Troop Leader. If the troop defaults or proper procedures outlined in this form are not followed, the person(s) completing this form is solely responsible for any uncollected debt. Due to council by January 2, 2025.

Parent Debt Procedure
This digital form must be completed and signed by the parent or caregiver accepting financial responsibility for Cookies. If the form is not completed, cookies must be pre-paid. A separate form must be completed for each girl. Due to council by January 2, 2025.

Delinquency Account Form
In the event a parent or caregiver does not pay for cookies by your troop's deadline, a completed Delinquent Account form is required. Failure to complete the Delinquent Account form by March 10 will result in the Troop Product Program Manager being solely responsible for any missing funds. Please see Troop and Parent Debt Forms for specific procedures.
Delinquency Request ACH Form
In the event a troop will not have sufficient funds to cover the first or final ACH withdrawal, an ACH Delinquency Request form needs to be filled out and submitted prior to the date of the ACH draft. Refer to page 26 in the Cookie Program Volunteer Guide for instructions on what is included and to whom this form will be sent.
Request for Cookie Booth Sale Dates (Digital | Print)
Council members, including Volunteers, can use this form to secure locations for our troops to use during the Cookie Program to sell cookies. Refer to page 18 of the Cookie Program Volunteer Guide for more information.
Cookie Entrepreneur (coming soon)
Ever wondered if a business owner in your hometown was ever a Girl Scout - well it never hurt to ask! Girl Scouts are encouraged to go to local organizations and talk about the 5 skills they learn by participating in the Girl Scout Cookie Program. Who knows? They might find others in the community who were Girl Scouts inspired by the Girl Scout Cookie Program to pursue entrepreneurship. This form will close on March 9, 2025 at 11:59 p.m.
Cookies on the Go
During the week of February 12-14, Girl Scouts become traveling entrepreneurs by taking it back to the basics by selling cookies door-to-door! Grab your cookies carts and wagons, map out your neighborhood or local business district and submit a picture of your Girl Scout offering delicious Girl Scout cookies to people in your area to earn this reward. This form will close on March 9, 2025 at 11:59 p.m.
Bling Your Booth
Girl Scouts of Historic Georgia is so excited to introduce a NEW Cookie Challenge to the 2025 Cookie Program! Each Girl Scout Cookie Booth weekend celebrate the sale with a themed table for a chance to be featured on GSHG social media and other marketing materials. This form will close on March 9, 2025 at 11:59 p.m.
National Girl Scout Cookie Weekend
It's time to Unbox Your Future and Embrace the Possibilities of the impact of Girl Scout Cookies. During the weekend of February 21-23, Girl Scouts can earn this patch by visiting local businesses to sell cookies and inspire the creation of treats and desserts using Girl Scout Cookies. Form submissions will be accepted until 11:59 pm on March 9, 2025.
Cookie Program Extreme Team Rewards
This reward is for our super selling Girl Scouts that sell 1,200+ packages during the Girl Scout Cookie Program. This separate JotForm must be completed by the Girl Scout’s parent/caregiver or the Troop Leader(s). Refer to page 31 of the Cookie Program Volunteer Guide for more information.
Product Program End Survey
We extend the greatest appreciation to our volunteers, parents and guardians, and Girl Scouts for a successful Fall Product and Cookie Program. Despite all the challenges, you have exemplified our theme of "Embrace Possibility" to the fullest. The following survey will help us improve next year's programming. Service Units must meet all requirements to earn $0.25 per case sold. This form is due no later than March 21, 2025 at 11:59 p.m.
Girl Advisory Sign Up Form
Sign up for our Girl Advisory Committee to share your ideas on all things rewards, programming, and even Girl Scout Experiences. The purpose of this meeting is to get feedback from our Girl Scouts regarding all things Fall Product and Cookie Program. If you have big ideas and want your voice to be heard, please complete this sign up form and jot down in your calendar our upcoming virtual meeting times and dates.
Product Program Planning Committee Application
Girl Scouts of Historic Georgia invites volunteers to serve on the council-wide Product Program Planning Committee. This committee will be comprised of Product Program representatives from each region to ensure diversity and all the council’s geographic regions are represented. We are looking for NEW committee members that will provide GSHG with valuable input for future Product Programs.
