Live Q&A sessions will also be available after each session.  All training sessions will be recorded and available for future reference.  Please watch the training video(s) and then complete the Cookie Training Quiz. The quiz is required to complete the training.

2019-2020 Cookie Forms
Before the Program
  • Troop Product Manager Agreement
For Troop Leaders & Troop Product Managers - Volunteers who want, or need, access to eBudde must complete this online form, be a registered Girl Scout Volunteer and have a current background check.

  • Withdrawal Authorization Form
For Troop Leaders & Service Area Managers (For Bonus) - A signer of thr troop account must complete this form giving GSHG permission to credit, or debit, the troop bank account.  Additional troop bank account documentation may be required if there has been any change in the leadership team or troop bank account since last year.  Troop bank account information should be entered into eBudde.

  • Troop Debt Procedure
For Troop Leaders & Troop Product Managers - Must be signed by the Troop Leader and Cookie Manager and returned to GSHG prior to participation.  Must be printed and signed by person accepting the troop product management position and troop leader.  If troop defaults, or proper procedures outlined in this form are not followed, the person(s) completing the form is solely responsible for any uncollected cookie debt.

  • Cookie Parent Permission Form
For Parents - Must be completed online by a parent or legal guardian.  If personal credentials, valid driver's license number and social security number are not provided, cookies must be pre-paid for.

  • Parent Debt Procedure
For Parents - Form must be signed by every parent with a Girl Scout participating in the Cookie Program and returned to GSHG prior to participation.  Must be printed and signed by parent or guardian accepting financial responsibility for cookies.  If form is not completed, cookies must be pre-paid for.
During and After the Program
  • Delinquent Account Form
For Troop Leaders & Service Area Product Mangers - In the event a parent does not pay for cookies by your troop's set deadline, a Delinquent Form is required.  Failure to complete the Delinquent Form by the published notification deadlines will result in the Troop Product Manager being solely responsible for any missing funds.  Please see the Troop and Parent Debt Forms for specific procedures.

  • Operation Cookies from Home Patch Order Form
For Troop Leaders & Troop Product Managers - Troops who had girls donate cookies to Operation Cookies from Home (OCFH) should complete this form in order for the girls to receive their patch.  Typically these cookies are virtually donated (council sends bulk cookies at the end of the sale directly to local military bases), however, some girls know military personally and opt to mail them cookies directly.  In that case, the cookies come out of the troop supply, are credited to the girl under the OCFH column and USPS form is needed to complete the form.

  • Gift of Caring Order Form
For Troop Leaders & Troop Product Managers - Troops who had girls donate cookies to Gift of Caring (GOC) should complete this form in order for the girls to receive their patch.  Typically these cookies are donated to local non-profits and first responders at the end of the program.  The cookies come out of the troop supply and are credited to the girl under the GOC column.  A picture of the donation is appreciated, but not required.

  • Cookies on the Go Form
During the first weekend of the 2020 Cookie Program, February 6th—9th, girls are encouraged to hit the pavement in their neighborhoods!  Take it back to basics before booth sales began and grab your cookie carts and wagons to offer Girl Scout Cookies to each person in your local community.  Attach photos of the girls in action to the online Cookies on the Go patch request form.  Girl must be pictured to earn the patch.  Photos may be used in social media and in council newsletters.  Cookies on the Go online patch request form will be active from February 10th-13th, 2020.  No late entries will be accepted.

  • Service Area Bonus
